In Angel Falls, is it that, e.g, Toma's mom's mind is in Index's real body, but the mom, and everyone else outside of Toma and the other sorcerers, thanks to the magic, sees nothing unusual and thinks it is her own? Or am I misinterpreting how it works and it is just the appearance that is being changed from the perception of Toma and the sorcerers, but no actual "swap" takes place - it's really just his mom's body.

It's just the appearances that are altered. A body swap would have caused unending panic, what with everyone waking up next to the wrong person or in a stranger's bed. Also, plenty of people would have been conscious when the change happened, so a changing of where they were a second ago would have been world-shattering. Easier for magic to fuck with the mind's perception. Really, "seeing" is not what most folks think it is. Our brains receive information from our ocular units, and that information is translated and ordered in our brains to fabricate what is perceived by our senses. So, if there were a mystical energy that could even lightly alter what information our brains use to tell us what is happening outside of our skulls, then we would be at its mercy.
