I haven't seen the whole reaction yet, but.... Shisui didn't make a mistake. The drug just has that effect. Taking it means taking a risk. Compared to dying for sure, it's the better option. Also, anything that would make you appear dead would also tend to do damage. Maybe it's lack of oxygen. The type of damage done seems to be nerve damage, which would be anything from tingling to total paralysis, also it could be brain damage. It's variable, but uncontrollable.

The only thing that the Shrine Keeper said about Luomen to Maomao in regard to the Shrine was that Luomen could have figured it out. Doesn't mean he did. He just knew that with the knowledge and the abilities Luomen had, he was equipped to know about color-blindness. I don't think the man would have told Luomen much about the Shrine because it would lose its function if the secret got out. ......
