I think Katherine really did love Stefan, but it was in a more self-serving kind of way. She loved the idea of being with Stefan and feeling human again and when he initially rejected her "other side" it made that yearning to want to be loved by him that much stronger. In the same way that I feel both Stefan and Damon want Elena because she can make them feel human and loved again, though Stefan is doing it in a better way where he gives Elena her own agency where as Katherine tried to "possess" Stefan. I think her feelings of self preservation outweighed her love for him, which is why she spent 150 years not contacting either of the Salvator brothers even though she knew they were alive
