I love this episode. It handles the ambiguity of what's real very well, and SMG sells it marvelously. With regard to your final comments - of course Tara showed up. Buffy was about to break out of her delusion, and the normal traps weren't holding, so her mind brought in some backup. The wise confidant with magical powers who could keep her safe.
