Wondrous item, uncommon (requires attunement by a barbarian)

While wearing this circlet, you gain 5 temporary hit points whenever you enter a rage for the first time during a combat. You also gain 1d4 + 1 temporary hit points whenever you slay a creature while raging during combat. You lose these hit points if you have any of them remaining when combat ends.

My hunger for battle boils over, but it is endless. With every victory, I am renewed, yet never sated.

Nice item! For my own campaign, I would tighten up the language to "reduce an enemy to 0 hit points" instead of "slay"; I don't feel like letting my players know if a foe is actually dead or not just because the barbarian got temp hit points. Might change "when combat ends" to "when rage ends" ... I haven't decided. As it stands, if the barbarian loses rage during combat, he keeps the temp HP and I'm considering whether it makes more sense for the points to fade if he's no longer raging. Also, since the temp points disappear after combat, I'd raise the total to 10 temporary hit points when you activate your rage, and increases the gain during battle from 1d4+1 to 2d4+2. For an attunement slot, this makes the item more attractive for those who have to decide which items to attune to and which aren't worth it. The net gain is only 3 more temp HP, but it looks more attractive on paper.

Totally fair! I'd just introduce this closer to level 5 if you go that route. I opted for "slay" here to avoid someone playing yo-yo to get THP (not like it's a big issue, but the potential *is* there technically). I always just try to close potentially frustrating loopholes so GMs don't have to make rules on the fly and be seen as the unfun spoilsports. That's my job!
