So to clarify on Willy Tybur. Their family were the only group of Eldians that went against King Fritzs wishes and helped all of the Eldians left behind on the continent while also helping Marley's economy. This is why they are the "True" leaders of the country even though they are Eldians themselves. They're considered royalty from outside countries POV and hide that they control things. That whole thing about hero "Helos" saving Marley was a fabrication. They made it up. No real Hero. The Tyber Family was more or less trying to atone for Eldians past actions while also making sure they were alive and enjoying the profits on the side. What this episode is setting up is Magath and Willy coming to an understanding that they need to clean out the whole regime and start over. Hope this helped a little!

A lot of this should be explained in the next episode actually. So maybe a bit of a spoiler in some spots, but more or less, yeah. To confirm that scene in the hospital ward, Eren was in fact talking to his own grandfather. Grisha became a doctor because his father was one. The grandpa isn't currently one though considering he's half passed crazy (with good reason). Grisha's being a doctor was part of the reason he was sought out a Restorationalist. Something I just recently put together myself, when Eren is talking to his grandfather did you catch Falco's last name??

I had to look it up:) Grice was one one of the restorationalists who was turned into a titan right? I didn't remember that...

Okay... This does give me hope that the "solution" is actually gonna be a good thing then:)
