❄ Holiday Cards Sign-Up! ❄ (Patreon)

Hey everyone!

As most of you know, there was a poll set up for you all to vote on whether or not you'd like to receive a Holiday Card in the mail from me this year, and overwhelmingly the votes were all YES! I've closed the poll a couple days early given the response, and am now presenting you with the form below ♡

❄ ❥ HOLIDAY CARD FORM!! ❥ ❄

The form will ask for:

Your Patreon username

Your name (to whom I'll be addressing)

Your mailing address

There is also a small section for any additional notes that you may have

The form will be open from November 19th, 2019 to December 3rd, 2019

Please be sure to fill it out if you are interested in receiving a card! I will be doing my best to include some extra goodies, so please be aware that the cards may not get to you exactly by Christmas (if you celebrate) and may even get to you in the new year.

Also, please note that I WILL be shipping ANYWHERE! So even if you are overseas, you are more than welcome to sign up for a card!

Thank you all for your interest! I really hope to send as many cards as possible to all my lovely Patrons  ♡  ♡  ♡

XO, AZULITY
