If a player is revived before having to make any death saves, they are revived with a point of exhaustion and can only heal to 75% their full HP until their next long rest.

They can, however, receive the benefits of any spells that grant them additional HP.

For each death save they make (pass or fail) before being revived, they suffer an additional point of exhaustion and have 25% less health (maxed out to 25%).

  • 1 death save: 2 exhaustion points and can only regain 50% of their total health until their next long rest
  • 2 death saves: 3 exhaustion points and can only regain 25% of their total health until their next long rest
  • 3 death saves: 4 exhaustion points and can only regain 25% of their total health until their next long rest
  • 4 death saves: 5 exhaustion points and can only regain 25% of their total health until their next long rest
  • 5 death saves: 6 exhaustion points and can only be revived through Revivify, Resurrection, or similar spells

Exhaustion Points Effects:

  1. Disadvantage on ability checks
  2. Speed is halved
  3. Disadvantage on attack rolls & saving throws
  4. Hit point maximum halved
  5. Speed reduced to 0
  6. Death

I like this a lot! It reminds me of a feature in Darkest Dungeon where if you heal someone with no HP, they stay alive but suffer penalties that make them very ineffective in combat, I think it's called Death's Door.
