The fifth amendment of t he US constitution protects people from having to make self-incriminating statements to the police or in court. "I plead the fifth" is a somewhat common expression in the US outside the context of the law. For example, if someone asks you what you did at some party the other night, you might say "I plead the fifth" to indicate that you're not admitting to anything, but you probably did something which might be considered "bad behavior". Basically the joke is Dr. Melfi's informal use of this expression with Tony is somewhat ironic given that mobsters will often have to plead the fifth if they end up in court. Also, I'm a mental health counselor. As far as I know, it's pretty rare for a MD like Dr. Melfi to do therapy. The only times I see psychiatrists practice is when prescribing and adjusting medications.

This is my first time watching the show and I don't get it. Feels like Seinfeld, or a reality TV show. We're just following people around waiting for something interesting to happen. Four hours in and the closest thing I see to an antagonist is Tony's mom.

I honestly couldn't get into it by myself despite a couple attempts. It wasn't until I watched it along with with a reaction channel that I enjoyed it. That being said, I think it's a very good show, but I don't think it's top tier in my opinion. I do appreciate your criticism though. It can be nice when people offer their differing opinions in a calm way.
