To save Mathara you just can't kill her. I usually try to put a hold on her and then kill everybody else around her and or use The knockout option.

Minthara IMO can be one of the most satisfying romances in the game, however if you want to experience her full romance and primary love scene, you need to kind of embrace evil a bit more than you may want. Rather than knocking her out, you need to *actually* side with her early on. This early scene has some elements well worth experiencing, particularly in a Dark Urge playthrough, but you do shut yourself out of a lot of other things in the game to get it.
