In Japanese language Ezra nickname is "Titania". Which translates to Queen in english. It's a nickname they gave her cause she is one of the most powerful in the guild, that's all it is just a nickname. It sound better in Japanese. One thing with Fairy tail is that the Japanese Sub version explains some thing better than english. Cause you watching dub, some things will be confusing not alot just some. and yes the eye this is the reason the spell didn't work on Ezra.

When they say Erza’s artificial eye weakened the spell they mean that because you have to look her in the eyes to be turned to stone, the spell was only half effective on Ezra since she only has one eye

For example, Natsu gets called “Salamander,” Elfman’s been called “Beast Arm Elfman,” Mirajane was once called the “She-Devil” (as Happy mentioned) and Gajeel’s been called “Blacksteel.” Erza, as one of Fairy Tail’s most powerful members, became widely known as the “Fairy Queen,” with Titania being a reference to the fairy queen of myth.
