Here’s another Patreon exclusive live model demo showing how I perform a pelvic floor rectal exam for conditions like pelvic floor dyssynergia, anismus, and fecal incontinence.

Dyssynergic defecation is when the pelvic floor muscles struggle to relax voluntarily, like when you’re trying to poop. Anismus is when the anal sphincters have a hard time relaxing during pooping or anal play, which can lead to pelvic floor pain, anal spasms, or that golf ball-in-the-rectum feeling. Fecal incontinence is the involuntary loss of stool.

In this live demo, I walk you through the whole process, from how I cue my patients to what I’m assessing when my finger’s up the bum. We go over muscle tone, coordination, sensation, and how I cue contraction, relaxation, and yes, even the “poop my finger out” instruction.

Give it a watch and let me know—how do you feel about an anorectal exam? If you’ve been nervous about one, did this help ease that at all? What questions came up for you after watching? Drop them in the comments.

Huge shout out to our model for making this video possible. ❤️

The breathing is helping, but I don’t like doing this stuff.

Anorectal exams don’t generally bother me. It’s all about technique. If an examiner just pushes their finger in without warning, it’s uncomfortable. I find breathing in and trying to relax works best for me. Once the finger is inserted, it’s no big deal.

As long as I have everything else tucked up out of sight, I've been able to deal with my anal area being examined. I've even twice consented to a mixed group of medical students watch hemorrhoids being banded and asked my pelvic floor therapist to do an internal just for assessment of my total pelvic health. Then of course, prostate exams. I did have an issue with admin folk being present when a urologist examined my prostate. I'm now wondering whether she thought having other males in the room as "chaperones" would make me more comfortable, couldn't find male medical folk so used the desk jockeys. Whatever, VERY bad move from my point of view.

Guy

By the way, I noticed the technical glitch, but just answered the question. 😁

😁

Orlando

Was this demonstration on a male or female?

Carlos

I don't get "the wrong video" technical glitch. Someone please explain.

Orlando

Got it. Thanks! Ive been in this position several times. Should I tuck my junk? Is that inappropriate?

Orlando

Thanks Dr. My concern is having/experiencing a spontaneous ejaculation. I guess if it happens, it happens 🤷 😊

Steve

Better watch out! If your model had Taco Bell for lunch, it may literally be "on you!"

I have just joined you. I have a complicated situation. It began following ligation of my rectal mucosa. I have rectal descent so my PT has said don’t do belly breathing and can’t do a drop because my pelvic floor is already at the bottom! What do you suggest? I also have dyssynergic defecation.

I have my first internal exam/manual therapy on Tuesday coming up and I'm super nervous about it. This video is super helpful.

Gosh, I wish I could offer personalized medical advice, but I'm unable without reviewing your individual case and performing a pelvic health assessment. If your pelvic floor muscles are weak either due to laxity in the connective tissue or weak muscles, it would be more appropriate to strengthen those muscles to support your rectal tissue. However, it is a balance between relaxation and strengthening which often requires coordination training not just focusing on strengthening. Rectal balloon training might be appropriate but I would defer to your treating therapist to discuss further.

Thank you so much for all you do. My first internal work was last night and it was professional and my pt made me feel comfortable for the whole thing. I experienced almost immediate relief last night and this morning following. I'm flabbergasted. My PT said we're going to be doing this going forward and I finally have some hope for relief from all my symptoms I've had for years.

I love your light banter and objectivity. You put to rest any hesitations I had about an anorectal exam. Are there any tests or way to monitor and evaluate an ejaculation objectively? I'm not finding much on this. Specifically, I can no longer complete an ejaculation. As soon as I start to ejaculate I get two share pains in the right ischiocavernosus muscle and that kills the rest of the ejaculation. After that the semen just seeps out.
