I don't think Loulan was pregnant, she was taking it as a precaution in-case she did get pregnant. But she could've been in an early stage of pregnancy at one point. The conversation in this episode implies that could've been the case. I think the insect to devour the emperor is implied to be her 'potential' child. But honestly I'm not sure since they were also planning an invasion. It also could've been about her assassinating the emperor by surprise after the invasion starts or something like that.
