So basically the medication vials are supposed to be extremely quick and easy to open because they need to be given fast. Santos not being able to get the top off quickly was a very valid concern about the vial being tampered with, and since Langdon had prescribed the medication and then returned it "unused" she thought he might have taken some of the medication.

Ohhh okay so he was injecting himself as well as taking the pills. Why would he return it after using it though… thank you for explaining, I appreciate it lol

Its not 100% confirmed that he was injecting himself (It could genuinely have been a problem with the vial) but it's definitely suspicious
