Derek was dying from this blue toxin when he grabbed Jackson's neck. Claws went too deep and this toxin poisoned Jackson. Doctor mentioned it. Argents were assuming that transformation could be from deep scratch but weren't sure. Also none ever mentioned 50/50 chance of turning. I think body very rarely rejects bite, but it can happen, so they just warning anyone who wants it, even if it is like 10-25% chance to die.
