Since you were confused about what was going on with Anthy in one scene: You missed a brief part that heavily implied Akio and Anthy murdered Akio's fiancée Kanae with poison (they were feeding her apple slices while she looked catatonic and barely conscious/alive with glassy eyes.) That's why Anthy uncharacteristically panicked for a moment when Utena later tried to go inside the room...because Akio was there presumably with Kanae's body. Also adds a little extra meaning to Anthy holding a huge saw in the scene (for making shaved ice, she says, but also probably to cut...other things.) Kanae also mentioned earlier in the Black Rose arc that her father (the actual chairman of the school) got sick after Anthy started visiting them. I thought that meant Anthy had poisoned him under Akio's orders, but the creators have stated that it was actually Kanae's mother (the woman who was secretly sleeping with Akio and did not like her husband at all) who did it after Akio's prompting. A little cultural context about Akio marrying into Kanae's family: when a family business doesn't have a male heir in Japan, traditionally a man is adopted into the family through marrying the family's daughter, so he can fulfill the role of the heir. So Akio got the power and the position he needed from the family and then murdered them when they were no longer useful to him. Well, maybe not the mother, but I really don't see him wanting to keep her around.

Also, a small detail: Nanami normally used two blades. Because in her original duel she was cheating and only interested in hurting Utena. Not this time.
