Utena falling off the horse was not planned, based on Touga's reaction. Akio just appears conveniently so he can look like a hero. Maybe with some magic involved. The roses aren't for flashbacks. Well, they CAN appear in flashbacks, but they're mostly in the present. There is a theory that Chu-Chu can represent Anthy's emotional state, since she is almost always hiding her emotions. For example, there have been moments when Chu-Chu is really annoyed by a bug or something the exact same time some other character shows up to talk to Anthy, indicating she finds that person annoying. Chu-Chu also eats in a way that looks VERY stressed out after Anthy was temporarily separated from Utena early-on. And, of course, Chu-Chu is scared of Akio.
