I dunno if you read Nalas original, but it was pretty heavily implied that Eijirou always had visual/audio hallucinations. During the first comic, most Charactrs thought it happened after a building collapse and that eh hit his head then, but at the end, Eijirou revealed that it had always been this way. It's like intrusive thoughts times 10 all those little negative things our brain tells us, Eijirou sees them pretty literally.
