Most people say the end revealing Johnny still being alive is just bad writing, but there are 2 theories that almost make sense, the first being that Johnny also sold his soul to be immortal, the second being that the pawnshop itself is a gateway to purgatory, and that's why almost everyone involved with the pawnshop either ends up dead or selling their soul.
