They seem to be trying their best at this point! All of this shit is hard to predict but I don’t really care for US socialists to compare this to what happened in the Soviet collapse. That was the largest decline in human life expectancy. Weve had a steady decline and that maybe ramping up. We’ve never been a socialist state outside of FDR’s new deal and some minor liberal reforms/regulations. What happened to that bloc of Soviet states is nothing we have ever seen in modern history. Because they went from A to Z. That’s not to say what is happening now won’t be devastating for a lot of people it’s just different in time and scope even if it feels like things that have happened in the past. Not trying to downplay this at all. I’m legitimately scared for people how reckless these freaks are.

Yeah definitely! I recommend Liz’s last crackpot article. Just stuff I was thinking about but she’s an incredible writer and puts things into a much more clearer focus. Things that have been jumbling inside my brain but reading that gave me less anxiety about what is going on to grapple with it because other people in this country understood what I was feeling based on what was happening. Just made me feel less crazy. I live in a very, very small town with tons of Trump freaks where the only mutual aid outlet is doing food pantry stuff for the local church.

Thanks Moe for mentioning SNF care under senior advantage plans. I left my job as a discharge planner for senior advantage at a SNF. People have no idea how they are being ripped off until they need the rehab and attempt to actually use their “100 Medicare SNF days” for rehab. What happens then is the patients get pushed into applying for Medicaid to cover more days at SNF and the burden gets shifted from the senior advantage insurer to the state. The insurance company keeps their money and the taxpayer picks up the tab. And the patient gets less “skilled” days with rehab and more custodial days (room and board basically)

Surprised but excited to see ya'll get into the health policy weeds of value based care vs. fee for service care. Interestingly, the House version of Medicare for All moves us back to fee for service (good). Bernie's version in the Senate maintains value based payments (not good).

People really don't understand how corrupt and evil Medicare Advantage is, largely because they haven't had to deal with it yet. My MIL has Lewy Body Dementia and a host of other chronic conditions and they constantly call her (and send her mail) trying to convince her to switch from regular Medicare to Medicare Advantage. Despite the numerous conversations and reminders, every year, one of them gets her to switch and then every year during open enrollment my husband and I have to get her switched back. In the meantime, its a nightmare because half the time she doesnt even remember switching and unlike with straight medicare, MA requires prior authorization for so many services and tests and if you get something done without it, they wont pay and wont approve any appeal. One of the most egregious instances was that she had daily physical therapy in assisted living because her neurodegenerative disorder has made her wheelchair bound and unable to move much. When she switched, the PT which was covered under Medicare didn't know and she didn't remember switching so she didn't tell them and for over a month they gave her services without knowing she was Medicare Advantage now and needed prior authorization for every single session. Flash forward to a $10,000 physical therapy bill from her facility MA refuses to cover yay. And we'll fucking be doing it again and again the next time one gets a hold of her and convinces her the freebies and perks are gonna be saving her money, really.

I myself do work in one of those call centers and I do want to kill myself. That being said I do inform all of my clients that prior authorization and referrals may be required for most services and would never enroll someone into a plan if they are in assisted living as that can cause them to longer be covered at that location. But at the end of the day, the agents are paid on commission and there are many who will say whatever it takes to get someone onto a new plan. The additional benefits can genuinely help some people but without being educated on how these plans works it be devastating I'm desperately trying to get out of the industry but am trapped in it for the time being.

Well, please don't! We need more people with enough conscience to think this way around, not less. I have never ever blamed the callers, I only blame the system and companies who have purposely set this up knowing it will create these situations, not the workers who are forced to carry out their dirty work. When I was a very poor breadwinner for my kids, I did Liveops from home and I realized they were taking advantage of senile old people to upsell. It made me sick but I also needed to feed my kids, as soon as I could I quit that shit and I went to nursing school and I believe over my career I did a good job advocating for my patients as a nurse, comforting the dying for comfort care/hospice, providing crucial services as an abortion nurse, and saving lives as an ICU nurse in addition to my personal political activism, I believe and hope I made amends for what I did to survive in a broken system. You, too, have a long life ahead of you with which to make up for things done in a broken capitalist framework to survive when you didn't have power to wield. We have to turn that negative emotion on the people in power, not internalize it when we are the very people who will be in the same vulnerable positions and they are confident they never will be.
