A cool way to start the story would be for Eddard and Robb to die before Brandon or Rickon were born, as punishment for Catelyn having prayed for Jon's death when he was a child.

And Jon, as Ned's eldest surviving son, would be legitimized by Robert and assume the lordship of Winterfell at the urging of the Northern lords, who would not want an Andal woman—a servant of the Seven—ruling the North.

A good starting point for the story would be the years preceding the Greyjoy Rebellion and the deaths of Ned and Robb—specifically, an Ironborn plot to sow discord in the North by depriving it of its Lord Paramount and heir, and to plunder timber from northern forests for their fleets while the eyes of Westeros are fixed on the Winterfell succession crisis.
