Idk, I thought it was easy to comprehend from the in-world point of view that Luffy looks mad different. Even as a viewer I can acknowledge that he really does look different. Maybe not completely, as he still has Luffy-esque features and such, but if you put default Luffy next to Gear 5, they look pretty damn different. His hair color and style are different, and his eyes are different as well. And a character isn't gonna see the hat from certain angles anyway. It makes significantly more sense than ppl at Gohan's high school not recognizing him as the "gold fighter" or w/e lol.
