This either calls into question Saki’s method of training her little brother, or is indicative of another problem. We know that Tomo can swim around pretty reliably for about five minutes, so time is certainly not an issue yet. You would also think that Saki would have prepared her brother for at least the shallowest of the shrine caves, which in this case means introducing him to depths of forty to fifty meters. Enough so that he doesn’t struggle with that aspect. However he clearly is here, so either Saki is literally throwing her brother in at the deep end, or something else is wrong. Could it be that whatever they decided to stuff his top with (fruit, softballs, anything round and sufficiently squishy, I guess) is interfering with his ability to dive? During training Tomo went topless, so he didn’t need to worry about a foreign object being pressed into his chest from water pressure. But now he begins struggling as soon as his outfit changes? They could have just decided to leave his top unstuffed. He didn’t lie about his age, and some eleven year old girls just don’t have “development” there yet. Point is: Saki is actively endangering her little brother by introducing foreign objects near his body without preparing him to deal with said objects under pressure. She also said she would make sure he comes back up, even if she doesn’t, but she’s actively making that harder on them both. Let Tomo dive topless!

If it’s emergency air the gods would be even more angry than if they found out he was a boy. The whole point of the two piece is to make him appear like a girl to appease them (which I still believe is unnecessary, at least with regards to his gender), but if you introduce something to breathe from that defeats the whole purpose of the ritual. Even they should be smart enough to figure that out. So no, I believe it’s highly unlikely for Tomo to both out himself as a boy AND take a breath, all in the same action, all at one of the holiest priestess sites. That’s GUARANTEED to make them upset. At least if he’s only outed as a boy there’s still the possibility of pleading his case considering who his mother is.
