Panels like the top of this are among my very favorite to create, because they're incredibly efficient without being crowded. In the foreground, Algy and Evans muse grimly over Tilly's murder while in the background one of the arrested patients notices the corpse gurney as they are led away. I will sometimes simulate depth of field or distance by using lighter tones in the line art, as I have in the first panel.  This way the foreground stays sharp and "pops" while the background doesn't compete while still being recognizableTilly doesn't have any practical reason to unload her gun here, it's almost more of a fidget as she ruminates on what she's done. The gaping hollow of the cartridge case is a small abyss for her to gaze into as she remembers Arthur's last words.
