The Charmed Ones, distracted by Leo's confession that he is an Avatar, allow a couple of demons to kill an innocent they were protecting, so Leo uses his powers to reverse time and vanquish the demons.

Do you remember Avatars from season 5? They tried to recruit Cole as well. And eventually they did. That's how Cole changed the past - when Paige sneezed and orbed. In that episode we saw how would everything turn out if Piper and Phoebe never met Paige. It's the episode when Cole finally died for real. Now here's Leo, tapping into the same power as Cole did. Well, Leo still has his Elder powers as you can see, so that's probably the reason why Elders didn't sense that he's also an Avatar. Plus, they're focused on looking for Avatars on the outside and trying to predict and sense what they'll do, that it didn't occur to them to check on the inside.
