You have to remember, when this aired sexual reassignment surgery was much less advanced than it is now. The funniest thing for me is how much South Park gets more "woke" as the series goes on (see this episode versus the season 18 episode "The Cissy"). That's why I don't think anyone should get offended by it. It's like a time capsule of the changing views of society (and Trey and Matt) over three decades. I would be surprised if no one has ever done a dissertation on it.

John, they will NEVER be able to change someone's chromosomes. Chromosomes are fundamental to our bodies: Each person is born with a unique set of chromosomes in every cell of their body, which essentially acts as the blueprint for development. Changing someone's chromosomes would mean altering the genetic code in trillions of cells throughout their entire body. Current technology does not exist, nor will it ever exist in the future, to achieve this on such a massive scale without causing severe harm.

I am well aware of that, but saying "nor will the technology ever exist in the future" is one of the most non scientific things you can say. Besides, you are discounting mind uploading AND epigenetics, which is coming. Not much even needs to be done, a y chromosome is an altered x chromosome. It will most certainly be possible one day (and even if it's not, mind uploading avoids the issue entirely).

When you were born, they didn't check your phenotype. They looked at what genitals you had. There are many people out there right now who are undiagnosed intersex. They have two X chromosomes. So if that's the definition you want to go with most of you have ALREADY had sex with a different phenotype than you intended.
