More about Tsurai's trauma is in Chapter 60, Wake Up. He has a dream featuring his mother lying there after a drinking binge. Also Chapter 82, Open Book. Re-reading now, I realize that he wasn't an open book, because he left the listener / reader assuming one thing but it fits another just as well. Re-thinking it, I suspect it is his legal given name. I can't find a good source on-line of All The Japanese Given Names. I can't find a page with Tsurai as a name element, but maybe I'm searching wrong. Furthermore, it seems cruel to name a kid "bitter, painful", which is why I assumed he'd chosen it for himself. (C.f. Mint, who is legally Min-Joon Park. I assume "Park" is his surname.) On the other hand, thinking about it now, It's hard to imagine Tsurai's father, Mr. Stick Up His Butt, calling him anything but his legal name, or *possibly* a close nickname. Something like "I refuse to recognize that word, whatever it is. Your name is Billy-Joe-Bobby, though I can't imagine why That Woman chose that name, and you WILL use that name." So I'm thinking now that his mother may have told the hospital "His name is Tsurai. It's a Japanese name." And everyone went "O.K.", and lacking Japanese people around to WTF at him, Tsurai himself didn't realize the meaning until later ... or ever. Anyway, I'm not sure.
