We first hear about Lalo and Nacho (Ignacio) when Walt/Jesse kidnap Saul. Saul, scared out of his mind, says "It wasn't me. It was Ignacio." and then a bit later asks "Lalo didn't send you?" So something happens/happened that includes those three that makes Jimmy scared of Lalo. As for Kim, I think she loves Jimmy, but is also stubborn. I think she knows that there is some truth in others, say Howard or Chuck, telling her things about Jimmy but she doesn't want to accept or face it. And of course, she does enjoy pulling cons with him and tries to convince herself that she's doing it for the greater good.
