This looks like the same way I did my bulge fabric for my zeeb. did you use a shrinkwrap modifier on a mesh that surrounded the genitalia?

hehehe no I used marvelous designer to create and simulate the clothing I used shrinkwrap in blender long time ago, but nowadyas I mostly do the clothign with marvelous designer
