if you actually watch the fight back, once Orihime and Chad show up, Tsukishima never actually swings on Ichigo at all. He raises his sword at him but he never actually attacks. So Ichigo looks like the aggressor here because Tsukishima never directly attacks him. And even before that, Ichigo instigates the fight once they get upstairs and Tsukishima doesn't do anything but defend himself, so to the ohers Ichigo just looks like he's attacking someone they care about with no real reason behind it. The only person Tsukishima actually attacks is Uryu, and that's only after he fires at Ginjo.

It's not as difficult as it may seem, for example. Tsukishima took Sora's place for Orihime, so to Orihime her brother is still alive, that's the reality she would have lived. Think about how her life would be different. I mean she only met Ichigo because Sora had that accident for example
