at this point in time, i remember thinking that clark kent is a captain. so whether he won or lost in a fight, people would be alerted by the fighting. therefore, he must have been killed by some deceit or backhanded way. as in not a fair fight

Also, When ichigo first fought ikkaku he says kisuke was his teacher and it cut to a scene showing kisuke in his captains clothing. That doesn't necessarily hint at him being Squad 12 Captain yet, but he does say he's a Captain all the way back then.
