If I'm not mistaken, the lieutenant was Ganju's brother.

The talk about honor is what Rukia had a flashback in the episode Ichigo fought a hollow in the graveyard.

This man seems to know when to pull out... Sorry, had to.

As sickly as he is, we are talking about a captain. So while he is weakened, we would be talking about the kinda difference we saw between Renji and big bro Kuchi. That is the whole rule about captains and lieutenants.

This coming weeks set of episodes is gonna be awesome!

Mr. Miles

As others have already said, Kaien Shiba was the brother of Kukaku and Ganju that was killed by a Shinigami. And for some reason, Ichigo looks just like him. Though this brings up the question of why Gangu and Kukaku never mention that Ichigo is the spitting image of their dead brother.

Mr. Miles

Oh and now you've got a mystery of why Ichigo and Kaien look so similar. Is there a reason behind it or is it just random chance? The fans wondered about that for a long time. I remember digging through old manga chapters, trying to find some clue. My theory was that Kaien soul was reincarnated into Ichigo. (When you die in Soul Society, you're reincarnated in the human world) However, i will not say if my theory was or wasn't correct. You'll just have to wait and find out what that explanation is, or if there even is one.

Notice that Kaien and Ichigo look very similar

I agree that Ichigo may very well be a reincarnation of Kaien. What happens when a Shinigami (Soul Reaper) dies is that they're reincarnated in the human world, with no memories of their former life. So Ichigo would have 0 memories of Kaien, if it's the case.

While it's not truly the illness, for all intents and purposes Captain Ukitake's disease is TB
