I’m trying eliminating all the moaning sounds and the occasional touching of the nipples to see if that can help pass the censorship

This is so crazy because I see so many ASMR videos that literally say "Nipple Stimulation" in the title of the video and they are up no problem. So dumb. Sorry you have to deal with this Sergio <3

Hmm. If they restricted it before it was even posted then not sure its the moans etc. I too see a lot of direct nipple stimulation and titles.. I wonder if its either the title or thumbnail. maybe the don't like the speedo or if the crop makes them look naked or something. Automated systems are a pain. but there may also be something to the comment about once you are flagged enough you are just on the list of those to scrutinize. But they should make it easier to argue. I have personally noticed improvement in music flags where there is just a link to context the decision and they use a better system to check. I seem to win everytime with that upgrade Even though they really aren't technically are you marking them as for adult only? I don't know how that affects monetization if at all.

It's complex. I think there are 2 types of restrictions for a specific video and they overlap. 1 is the yellow dollar that means "Limited monetisation" That means they think the video is not suitable for ads but it can still get money from YouTube premium users (which is very little) The extreme of this restriction is shown with a red dollar and it happens for instance when you don't have the rights of a song. And the other restriction is +18 years old. This one has the same limited monetisation and also ensures that people who want to watch it have to log in (parental stuff I guess) BUT THERE'S ALSO A HUGE DIFFERENCE BETWEEN BOTH OF THEM and its the fact that with the limited monetisation your video is still pushed by the algorithm. In my case this is important because even if I don't get almost any money from the video, it's still very good for advertising my Patreon and get new patrons. But if I get the 18+, then Youtube doesn't show the video to anyone so it's just useless to have it.
