Who Elena and Rebecca saw wasn't really who they were thinkign about the most. It was a subconscious manifestation of their inner fears. Elena is scared that she might LIKE being a vampire. Which is why she sees Damon... someone who likes being a vampire. Sadly Stefan here HAS placed some guilt and shame on being a vampire by not allowing her to learn to feed on people. He doesn't trust that she's strong enough to get through it because he wasn't. It's projection and he just doesn't realize he's doing it. Rebecca's worst fear is that she doesn't deserve to be loved because as her subconscious was saying she's had a 1000 years to make herself a better person and hadn't bothered yet. Hence Matt appearing to her. She thinks she doesn't deserve someone like him.

Oh and Caroline and Stefan, I love that scene. I love all their scenes. Stefan just always looks so happy around her

Well, what is also the one thing she hasn't want to admit or accept consciously? Her feelings for Damon admitting and accepting them, so another reason he appeared since it was her subconscious trying to get her to accept those feelings.. When she made her choice, it was made from more a conscious standpoint in the moment, she was making a choice based on she felt really in the past concerning Stefan, but not what she had been feeling for Damon in the present. We as an audience saw all she was doing concerning Damon from her subconscious stand point, getting Jealous over Rebekah, caring for him, and all the other minor actions you take when you actually love some one through out season three, that come from a subconscious level.
