So, what happened is essentially: "our" Okabe wanted to time leap, the time leap machine didn't work for some reason, they couldn't figure out the reason for the magical 48 hours, but they still had the memory data of Okabe from when the machine failed on a computer -> Okabe continues working with Daru up to when he gets tortured etc because he couldn't leap in time -> Okabe is left in a basically vegetative state due to some form of torture and Daru and so on rescue Okabes still alive body and keep him alive on machines -> Daru digs through his computers and finds the old memory data from the failed time leap of our Okabe -> Vegetable Okabe gets the memories of our Okabe transferred and we are at the beginning point of the episode that is pretty much what was said in the episode "Our" Suzuha still went with Mayuri, but that does not prevent her from being born. That's the reason why there's still a Suzuha in 2036 who is the same age as before, Otherwise there would have to be 2.

Also, this episode is set before the time machine in the future is completed, so Suzuha hasn't traveled to the past yet and doesn't know Okabe (we can actually see the time machine while Okabe is talking with Hiyajo towards the end of the episode). As far as we know both Mayuri and the Suzuha we know are gone from when the time machine was destroyed in 2011

I don't remember if there is an answer for the error with Time leap machine. So my theory is he can't Time leap because the phone network didn't working (due to the terrorist) at the Time Okabe want to arrive. When Okabe leap, memories arrive by a phone call, so no possibility to receive a call, no Time leap.

Yeah this is sort of the point in SG0 where it starts to teeter under its own weight. We do know for sure that Operation Arclight did not succeed, since if it had, Okabe's reading steiner would have activated when they went back. This is the same as in the original when Suzuha went back alone and nothing changed. Their actual fate is unknown.

If that's the case, then time leap wouldn't have happened on the first iterations of the time leap.

I've compiled a list of the answers for the questions you asked and details I think should be clarified. project arclight seems to have failed due to the destruction of the time machine, our suzuha + mayuri may or may not be dead, but they are at the very least missing (i want to emphasize that the suzuha that we see in this episode is NOT the suzuha we saw enter the time machine in 2011) the time leap at the end of the last episode was not successful and we as viewers were only led to believe that it was for the sake of this twist the suzuha in this episode is not the same suzuha we knew in 2011, this suzuha has been born and raised after the timeleap failed and has never been to the past I don't know if they ever state specifically why the time leap failed, i just chalk it up to too many factors changed with this attempt and not enough care was able to be put into the creation of the device The fact that okabe is still alive is a very important detail in the mind of 2011 okabe, as it shows that even convergence can have loopholes. showing us that just because someone is "dead" does not have to mean that they are dead, only that they are effectively dead in the grand scheme of the worldline. similar to how kurisu didn't actually have to die at the end of steins gate, just be perceived to be dead by that okabe so that his actions would be based upon that fact. now okabe at the end of the episode plans to use the time leap machine to get back to 2011
