this episode always gets me, too. Part one set up everything so well, that the emotional beats in the second part just feel so natural. This is also, in my opinion, one of David Tennant's best performances. He pulls off the tragedy of John Smith being overwritten so convincingly, that we genuinely feel sad for him. He goes from scared, to sad and worried, to cold and merciless in the span of 45 minutes. And it's done phenomenally.

Have said it in previous comments but this 6 episode spurt from Episodes 8 to 13 of this season are phenomenal... glad you've enjoyed the first two so much. There is an equally good, if not better, 6 episode run from Episodes 8 to 13 of Season 4 too.

Also the next episode is probably the episode with the most reactions on Youtube of any Doctor Who story, apart from maybe Rose.
