Sam would do all the things in the song for Dean. He'd put his fingers down his throat if he swallowed anything evil; he'd give him a blanket if he shivered; he'd let him wear his coat to keep him warm. Sam still dreams of a life for Dean where he's free, to feel the sand between his toes; he's willing to do what it takes to achieve that, to override his conscience and make it feel empty. Fitting for Cas's grace to be hidden in Don Quixote, as he's spent so much of his time 'tilting at windmills' - unwittingly fighting for an unworthy cause. But he shows again here that he'd be willing to do the maddest thing a man can do and let himself - the man he's become - die in order to serve the cause of protecting Sam and Dean.
