Trans people are treated fairly well, and their genders are always respected. About 5% of the narakyian population is some variety of trans. However it can cause controversy with older narakyian who especially expect their kids to fill certain gender roles or birth heirs.

RIght now they're not together and not in love per say, they're just good friends who do have some developing feelings for each other. He doesn't feel bad about having sex with others, it's just natural for him.
