0.0.10 "Stay Fit" update will introduce a new mechanic called "Blackmail".

Since most events can be unlocked by having a high-enough "trust" from a character, that completely cut off players who like to antagonize people rather than befriend them.

So, if your "Trust Level" with someone is low enough you can unlock new "naughty" conversations with them, which could eventually lead to blackmail them using your position as Chief General Practitioner.

This is but one of the few situations that will unlock by using this feature in the upcoming update. The rest will be up to you to find out.
