You should be able to grab your sword without taking your eyes off your enemy. They shouldn't hinder your arm grabbing it. They will just smush out of the way, unless they are fake.

Having taught several ladies with large breasts to fence, I modify their stance and some of their actions to account for the issues of such "assets". In the end, fencing is for humans, so I bend things to suit the humans, rather than bending them to suit fencing.

Absolutely. It is one of the reasons it is important to do at least a chunk of your practice in the armour you're going to wear, so you can get used to its movement characteristics. Not just chests, but also arms and legs, and it's not just the weight...
