This is the way alcoholism works. In so many cases it is nothing but slow and sure self-destruction. Combine it with clinical depression and it gets even better. Add undiagnosed Diabetes 2 on top and you have an early grave. That codependent relationship between Billie and Ruth is sheer genius when it comes to the rampant malice of life, mind you.
