My theory is that Sirius is not Fortuna. She was Geuse’s childhood friend, and being cocked by Fortuna. After Fortuna’s death, Sirius stole Fortuna’s hair/eye/ear from her body by some surgery, so that Geuse could love her. This might explain why: 1. She’s in bandage 2. She actually hate Fortuna/Emilia’s appearance 3. Geuse seems not in love with her.
