Gemma’s scar is from heart surgery. She mentioned it in one of the first episodes when she said Abel’s heart condition was the family’s curse. Also the actress had heart surgery in real life so the writer of the show, her husband, probably wrote it in because with the tops she wears, it’s hard to hide the scar.
