New form to provide more info about your shop. (Patreon)

we made a form where you can accept our terms and conditions, where we provide you resources and you can share the address/link of your shop, name, and provide other information so we can keep tracking our Merchants a bit more. 

As you can cancel your subscription any time we keep this information only as long as you are subscribed to our merchant tier, but please keep in mind that if you cancel you lose the right to sell the printed result of our products and you have to fill the form again if you re-join later.
