Classic symptom of shock in the bottom panel - a complete disconnect with time appearing to slow down from the sudden adrenaline surge, right after the event takes place. Note the visual 'shockwave' effect of the gunshot sound in the background, along with the still-present muzzle flash.

Oh, PTSD flashback. Good call. The clothes he wears in the first panel are the same as when he's looking in the mirror on the cover, suggesting that's what he's still doing while reliving what happened up to that point.
