Ling wasn't really being literal with that "monster hidden in its belly" line, it was just his way of interpreting what he just saw, not being an alchemist and all. Basically, all of the Homunculi have secondary powers (Greed's shield, Lust's nails, Envy's shape-shifting) and this is Gluttony's.

This thing with Envy and Marcoh illustrates a concept called the 'Monkey Sphere'. It's difficult for people to have real feelings or attachments to people they don't know or who aren't in their direct physical or social vicinity at least when it comes to weighing options like this. You can think of it like the Trolley Problem, except the smaller number of people on one train is your entire family. Makes the choice a lot more understandable if you pick the smaller amount.
