Is it a difficult task to make versions of these types of thongs to sit high on the waist? And more high wasted thong options?

Well, to be honest - yes. It's not about "just moving some geometry point higher" but making those point moves with the according bones & correctly interacting with the body shape. It's a tedious task and I would prefer just to use another source model that was already made and rigged for the high waist.

I'm guessing it would be insanely difficult to allow for wardrobe malfunctions over the course of a fight? Like certain moves creating a chance that a boob pops out of a bikini top, a nip slip, etc.

To be completely honest - it’s technically possible. But it’s a lot (like A LOT) of work for a quite minor effect.
