In the Japanese version, piccolo mentions goku is fighting sloppy and rushing with heavy attacks. When goku turned Super he increased the virus strength as well. So goku really rushing to win before it stop him.

Time travel in fiction wil always be massy. The best way to explain is trunks visiting in past and cell playing a part ( you wil find out later) create new branch of time line. Theres plenty of vidoes explained time travel and time lines how they work in dragon ball. Whis also explains how it works in dragon ball super in goku black arc. Its like mcu not the same thing but its similar concept of branch timelines

The butterfly effect does get confirmed but unlike domino effects their outcomes are chaotic and unpredictable. 2+2 can equal 7 doesn’t have to make sense type shit. The only difference in the timelines is who fights frieza and how long goku is in space for. No matter what he would trained just as hard for 3 years because that’s what he does.

Pretty much what happened is the virus showed up earlier than what trunks originally told them. So, yes, they knew about the virus and had the antidote. However, they thought they had more time before Goku gets it based on the original time trunks gave them.
