So in this episode we learn that Ishtar was able to manifest in this grail war because she dissolved a fragment of her divinity and personality into the world and set the condition that if Gilgamesh and Enkidu were to ever meet each other on earth again then that fragment would automatically awaken to “protect humanity”. That last part in quotes is just some bullshit she said the real reason she set that condition was to merely get revenge on Gil/Enkidu. As for why John was able to come back to life and get his arm back they vaguely explained this last episode but it was due to False casters Noble Phantasm which is called musketeers masquerade which pretty much allows him to rewrite fate to a certain extent
