I wonder if the dialogue means Ethan understands Malthus is trying kill him to mock Stryder for loving a human, or if there is a possible added element that Stryder is having this nightmare as well due to the transference. The last scene from the sanctuary didn't show Al in a trance, and I wouldn't be surprised if that changed once Malthus upped the ante.

I feel, that the only hair raising, bite my fingernails, nearly shit my pants, “😳😵😵‍💫🤯😱”, tiny detail to enhance my reading this episode; would be to display a small clock, ticking down the minutes and nano seconds as a “WTF?!!!!! LOOK AT THE TIME, ETHAN!!!” Just sayin’… 😅

I read this as Ethan sees that Malthus is constantly hurting Al. Ethan sees by himself that Malthus is extremely abusive - before he had only Al's report and now he knows that this type of behavior is going on for long, long time.
